Community Vaccinations

OU Health is proud to partner with the Oklahoma State Department of Health (OSDH) and OKC-County Health Department to distribute vaccinations to those in our community. If you have been scheduled for a COVID vaccine with us through the OSDH website, we know you likely have questions regarding your appointment. Location Information

• Jan. 23, 2021-Oklahoma Children's Hospital OU Health, 1200 Children's Ave.

  • You must have a confirmed vaccine appointment prior to arriving. First dose appointments have been scheduled for Jan. 23, 2021. Complementary valet is available or you may park in the P2 garage in designated COVID-19 spots.

Once Your Appointment is Confirmed

What to Bring

(Please have the necessary paperwork printed and filled out to bring to your appointment)

• Driver license or other state or government issued identification
• If first responder or medical personnel, bring identification/work badge/proof of credentials
• Insurance card or copy of front and back to provide registration staff
• Completed paperwork (all must be completed and brought to your appointment):

  • CDC Pre-Vaccination Checklist for COVID-19 Vaccine (English pdf)/(Spanish pdf)
• If you normally carry an EpiPen, please bring it with you.

How to Prepare

• Masking is required.
• Dress appropriately for easy vaccine administration. You will be asked to expose your upper arm for the injection.
• Please bring any assistive devices needed (cane, walker, wheelchair, etc.) For social distancing and the safety of all involved, visitors are limited to only 1 adult escort for patients with mobility limitations.
• Please arrive no earlier than 30 minutes before your scheduled vaccine appointment time. Entry into the facility will be screened for compliance with scheduled appointment times.
• For a seamless experience, please bring all completed paperwork, ID and your insurance card.
• Make sure you eat, drink and take routine medications before your scheduled appointment.
• After the vaccination, please be prepared to wait 15-30 minutes for observation.
• Make plans to return for your second dose of COVID vaccine. The Pfizer vaccine second dose is administered 21 days later. The Moderna vaccine second dose is administered 28 days later. You will return to the same location.

Billing/Cost

• Vaccine is FREE to the patient with no out of pocket cost.
• OU Health will bill insurance for the administration fee only.
